BDD Claim Required Documents
Benefits Delivery at Discharge (BDD) Claims can be filed 90 -180 days prior to an Active Duty service member's date of separation. The list below details the required documents you will need to submit with your BDD Claim.
- Active duty medical records - also referred to as Service Treatment Records (STR)
These can be requested at your Medical Group Outpatient Records office. A disc will be provided and we have the ability to pull the records from the disc. For dental records, visit your Medical Group Dental Office.

- If you are filing for PTSD, the VA Form 21-0781 is required - Click here for the form
- Voided check or deposit slip
- Birth certificates and SSN Cards for dependents (if applicable)
- Marriage Certificate (if applicable)
