Open Enrollment
Open Enrollment began October 6th and will end November 2nd.
If you want to make changes to your health, dental or vision plans, including adding or dropping dependents for 2025, this is your opportunity to do so. Changes made during open enrollment will be effective January 1st.
As with previous open enrollments, if you don't want to make any changes, you don't have to do anything. However, we highly recommend reviewing your current coverage and dependents to ensure everything is accurate.
Changes must be submitted via the Open Enrollment form. This form can be completed on your computer with a digital signature and emailed to Human Resources, or printed and hand written. Forms are due to your Human Resource department by Monday, November 4th.
New York Life has also agreed to allow us an open enrollment period for term life insurance. This is term life insurance, which is different than the whole life representatives were here to talk to us about this past summer. These policies have lower rates and typically changes are only allowed if you experience a qualifying event. They have created a special open enrollment portal for us with valuable information about the options available. It can be accessed via the New York Life link in Additional Information.
If you'd like to make changes to your term life insurance, like adding to your existing coverage, please fill out the form below and turn it in to Human Resources by November 4th also.
Another exciting bit of news, we are changing our supplemental vision insurance provider from Avesis to EyeMed. EyeMed offers a broader coverage area with more doctors and clinics accepting their policies. Additional information can be accessed via their virtual benefit fair site. Click the link in Additional Information and enter the password FY51WPQK.
